We should introduce polypharmacy reviews and ensure that they are not exclusive to care home settings. (Polypharmacy reviews are important for people who are regularly prescribed five or more medications. Their aim is to ensure that the medications are effective, the patient isn't experiencing side effects and that all the medicines the patient is taking are still necessary.)
